Hay a say on healthcare plans for the elderly in Notts
Nottinghamshire County Council is working with the six local Clinical Commissioning Groups (CCGs) covering the county to make sure that health and social care services are efficient and joined up wherever possible to improve the way older people receive care and support.
The Better Care Fund 2015/16 Plan will enable the partners to pool £59m of their money together to connect health and social care services more effectively.

Hide AdAll the services within the plan aim to support older residents to live independently as possible as well as reducing the numbers admitted to hospital and care homes, reducing delays in transferring people between services, increasing the number of people remaining at home with the support they need, and ensuring more services are available seven days a week.
